New Patient Registration
We currently have an open list and welcome requests for registration from patients living in or moving to the practice area (roughly a seven mile radius of Helston, please check with reception to ensure you are in our practice area). You can register by completing a form ‘GMS1’ which is available at reception.
The registration pack includes a brief questionnaire for each member of the family. This tells us a little bit about you whilst we are waiting for your notes to arrive from your previous surgery. Please return the completed questionnaire to the reception. It is confidential and will be included in your medical records.
You are able to express a preference of doctor and whilst the practice will try to comply it might not always be possible.
We encourage all new patients over the age of 5 to have an initial health assessment with the practice nurse. She will check your blood pressure, height, weight, and test your urine.
Having chosen your preferred doctor you should try to see the same doctor for follow-up of any continuing problem. In the case of an emergency, you will be able to see any available doctor.
Medical treatment is available from the date of registration. Please contact reception for further information.

We are increasingly aware that for many patients the best form of communication is via mobile phone and email. As a practice we would like to encourage you to register your details for these services.
There are several electronic services available:-
the simplest and best way to contact the surgery for consultation and prescription requests
Email and text communication
If you consent we can send you communication via email or rather than letter e.g. recall for clinics and results.
We can also send a variety of text updates including:
Appointment reminders to your mobile phone
Health campaign reminders e.g. flu vaccination due
All services require an independent and secure contact for each patient.
